Athletics Registration

Athletic Registration on FinalForms

Registration will be completed online for all sports on FinalForms.

How to Register for Athletics Online

Registration for Eagle Athletics:

  1. After parents/guardians follow the instructions below to create a FinalForms account, students will need to check their email for FinalForms new student account set up by parent/guardian.
  2. Students log in and sign the required forms.
  3. Students pay $25 ASB fee (once per year) online or in person at the school with Ms. Clark (Rm. 1113). Preferred method is online payment.
  4. Provide ASB receipt to Athletic Department via email: dgjurdy@seattleschools.org
  5. Physicals are required every two years. Student athletes provide physical form to Athletic Department by uploading to FinalForms or emailing a copy to dgjurdy@seattleschools.org

How to Create a FinalForms Account

If you previously had an account, you can log in and all your info will still be there! If you have not created an account, please follow the instructions below:

  1. Parents/guardians, go to the FinalForms website
  2. Click new account under the Parent Icon
  3. Type your name, date of birth, and email and then click register
  4. Check your email for a FinalForms email, and click confirm your account in the email text.
    • You will receive an email within 2 minutes prompting you to confirm and complete your registration. Remember to check your spam folder. If you still do not see the FinalForms email, please email support@finalforms.com.
  5. Create your new FinalForms password and click confirm account.
  6. Click register student for your first child. Add your student and their email.

Additional Forms (if needed)

  • District Physical Participation Form – Sports Physical Expiration date has been added to FinalForms accounts, you can check there for most recent physical date.
  • WIAA Running Start Contract (link posted soon) – required for Running Start students only.
    • Note: An attached copy of the College Schedule is also required.
