Registration Forms
Any student enrolled at Cleveland can be seen by our providers by having a parent or guardian complete and sign the registration forms.

Students can self-register for confidential care.
THC Services
Our SBHC has mental health providers, a medical provider, and a clinic coordinator. We accept insurance, but services are provided at no cost to families.

Accessing Immunization Records
Refer to the Washington State Department of Heath website for instructions on how to access your immunization records without visiting your medical provider office.
