Graduation Requirement Checklist

This form details Cleveland High School’s graduation checklist for the graduating classes of 2021 and beyond. Included in this form are the English Language Arts and Math Graduation Pathway flowcharts.

Service Learning

One of the graduation requirements for Seattle Public Schools isĀ 60 hours of Service Learning.

Service learning is giving time, without pay, to a non-profit or government agency. Service learning allows students to learn and to apply academic, social and personal skills through activities. Students make a difference and address authentic community needs.

As part of service learning, students are asked to reflect and report on their experiences. You will find a list of service learning opportunities below. If you need help finding more opportunities, please contact your school counselor.

PE Waiver

Entering 9th graders may not request a PE waiver for their first semester. Participating in athletics does not guarantee a waiver is approved.

CTE Two-for-One Request Form

This form is to be used by students who earn CTE credit as required for graduation by taking a course that is cross-credited in a non-CTE subject that is also required for graduation and vice versa. Students may count the course they took as meeting graduation requirements in both subject areas.

Middle School to High School Credit Form

High school courses 8th grade students took in middle school, during the 2019-2020 school year, will be added by default to high school transcripts when students transition to high school.

High school courses taken during middle school prior to the 2019-2020 school year may be added to high school transcripts. Students may use this form to request courses be removed from or added to their high school transcript.
