Attention parents and students
The COVID primary and booster Pfizer vaccine is available in the Teen Health Center (THC) to all students age 12-18 years old. Please call 206-263-2608 or stop by the THC to schedule an appointment. Here are the COVID registration and consent forms to bring with you to your appointment:
If you are under the age of 18, the forms be complete and signed by parent/guardian.

Registration Forms
Any student enrolled at Cleveland can be seen by our providers by having a parent or guardian complete and sign the registration forms and returned to the SBHC.
Students can self-register for confidential care.

THC Services
Our SBHC has a Mental Health Provider, Medical Provider, and a Clinic Coordinator. Most, if not all services are free and many can be confidential, if necessary. The services that we provide are:
