Registration Forms
Any student enrolled at Cleveland can be seen by our providers by having a parent or guardian complete and sign the registration forms.

Students can self-register for confidential care.
THC Services
Our SBHC has mental health providers, a medical provider, and a clinic coordinator. We accept insurance, but services are provided at no cost to families.
