Cleveland Teen Health Center

Our Teen Health Center is a FREE service for students to get medical health care and is operated by Group Health, with funding support from the City of Seattle’s Families and Education Levy. For routine care, students need to complete a registration paperwork with their parent/guardian’s signature. The Health Center is staffed full time by a Nurse Practitioner, a Mental Health Counselor, and a Clinic Coordinator. Please note this signature is not needed for confidential care.

When is the clinic open, and how do I make an appointment?

Any student enrolled at Cleveland can be seen by our providers by having a parent or guardian complete and sign the registration forms and returned to the teen health center.

The clinic is open 7:30 a.m. - 3:30 p.m., Monday-Friday during the school year. Appointments can be made over the phone (206-296-0536) or in person with the THC Office Coordinator.
